TRANSPORTATION CHANGE & REMINDERS FOR 08-09 SCHOOL YEAR

ALTERNATE BUS STOP FORMS
Due to several serious safety issues with having multiple alternate bus stop forms per student we are making some changes to the current process which will provide a more safe and secure service for our students. This change is comparable to what other districts offer their students. Notifying families now allows ample time for families to make arrangements before this goes into effect for the 08/09 school year. Allow three (3) business days to process your request.
*NEW - Bussing schedule will be the same everyday – Monday through Friday.
“Alternating days”, “certain days of the week” or “as needed basis ” will no longer be options.
NEW – Options offered: (You may choose only 1)
1. Pick-up at HOME with drop-off at DAYCARE/SITTER.
2.
Pick-up at DAYCARE/SITTER with drop off at HOME.
3.
Pick-up and drop-off at DAYCARE/SITTER.
*The NEW Alternate Bus Stop form will be available on the Crestwood website by 5/1/08: at crestwood.sparcc.org, click on information, click on district forms, click on transportation – locate form.
*NEW - Only 1 Alternate Bus Stop form per child. (More then 1 form per student on file will no longer be an option.)

Alternate Bus Stop forms are only good for the current school year and you must fill out a new Alternate Bus Stop form each year.

If you know your arrangements before the end of the 07/08 school year you can fill out our NEW form and return or fax it (see form) to the Transportation office by 6/5/08.

If you plan on stopping in after 6/5, summer hours vary so please call ahead. (330.274.2776)
REMEMBER - TO REGISTER YOUR 08/09 JUNIOR OR SENIOR FOR BUS TRANSPORTATION
If you have a Junior or Senior that will be riding the school bus for the 08/09 school year please see below:
* Juniors & Seniors must register for bussing for the 08/09 school year.

Look for the 08-09 Junior - Senior bus registration forms to come home with your Sophomore or Junior in May.

Please return or fax (330.274.3706) the Junior - Senior bus registration form to the Transportation office by June 5, 2008. If after June 5th please reference our website or contact the Transportation Dept.

Registration forms will be available on the Crestwood website 5/1/08: go to crestwood.sparcc.org, click on information, click on district forms, click on transportation – locate form. You may also pick up 08/09 Junior- Senior bus registration forms at the High School or the Transportation office.

REMEMBER – THIS IS A REMINDER OF HOW BUS PASSES ARE ISSUED AND WHAT INFO IS NEEDED
* As always bus passes are issued on an emergency basis only. Approval is issued from the building administrator, secretary, or Transportation dept.
*A detailed note must be sent or a phone call to the school office from the parent or guardian with all pertinent information to have a bus pass issued. Information needed: Name, GR., Reason, Bus #, and Name and Address of person at emergency stop.
*Sleepovers, parties, school projects, carpooling, evening groups, clubs or lessons etc… are not considered an emergency and will not be authorized.
*If your emergency arises after school hours and will affect the morning route please contact the Transportation office at 330.274.2776. Feel free to leave a message.
* Bus passes will be monitored for abuse.
